Change of Course Level Form

This form is to be used to transfer courses taken while enrolled as an undergraduate or professional (Law/Vet Med) at the University of Illinois towards a graduate degree.

Instructions for Completing the Form:

Student’s graduate department office must review the courses requesting to be transferred. If the department agrees they are eligible for graduate credit, please sign in the Department Approval section of the form.

Other approvals necessary:

• Signature from College Releasing the Course
• Signature from College Accepting the Course

If the request is approved and once all necessary signatures are obtained, the form will be routed to the Office of the Registrar for final processing.

Change of Course level is considered "Transferring Credit from outside the Graduate College". These requests are subject to the Graduate College policies regarding transfer of credit.
